Mesothelioma Chemotherapy-A Basic Overview

Chemotherapy is one of the commonly used conventional treatments for mesothelioma cancer. Chemotherapy is cancer treatment with anti-cancer drugs. There are 2 main ways that anti cancer drugs can be administered:

1- Chemotherapy can be given systematically. In systemic therapy, the anti cancer drugs are swallowed in pill form or injected into a vein. The drug enters the bloodstream and circulates throughout the body to get to the cancer cells wherever they may be and kill them.

2-Chemotherapy can also be administered locally. In this form of delivery, the chemotherapy drugs may be given intrapleurally (directly into the chest cavity) or intraperitoneally (into the abdominal cavity) through a small incision in the chest or abdominal wall. This approach may allow doctors to give higher doses of chemotherapy to the tumor while limiting the side effects to the rest of the body. Chemotherapy drugs given this way may be heated first (hyperthermic chemotherapy), which may help them work better.

Chemotherapy may be given as an addition to surgery; it could be administered before or after the surgery. When it is given before the surgery it is referred to as neoadjuvant therapy and when it is given after surgery it is referred to as adjuvant therapy.

For advanced tumors that are not operable, chemotherapy can be used as the main form of treatment (alone or along with radiation therapy). Chemotherapy may slow the progression of the disease, but it is very unlikely to give a total cure.

Chemotherapy is given in cycles, with each period of treatment followed by a rest period to allow the body time to recover. Chemotherapy cycles generally last about 3 to 4 weeks. Pemetrexed interferes with levels of folic acid and vitamin B12 in the body, so these must also be given to avoid certain side effects.

Other chemotherapy drugs used to treat mesothelioma include:
• gemcitabine
• carboplatin
• methotrexate
• vincristine
• vinblastine
• mitomycin
• doxorubicin
• epirubicin
• cyclophosphamide
• ifosfamide
These drugs are usually given in combinations of 2 or more, but single drugs can be used in people who may not be able to tolerate combinations of drugs. Several other drugs are also being studied to determine their effectiveness in treating mesothelioma.

Side effects of chemotherapy drugs

The mode of action of chemotherapy drugs is to attack rapidly diving cancer cells in the tumor growth, but in doing so they also attack other normal rapidly diving cells of the body like those in the bone marrow, the lining of the mouth and intestines, and in the hair follicles. These cells are also likely to be affected by chemotherapy, which can lead to the development of side effects.

The side effects of chemotherapy depend on the type and dose of drugs you are given and on how long they are used for. These side effects can include:
• hair loss known medically as alopecia
• mouth sores
• loss of appetite also known medically as anorexia
• nausea and vomiting
• increased chance of infections (due to low white blood cell counts)
• easy bruising or bleeding (due to low blood platelet counts)
• fatigue (due to low red blood cell counts)

These side effects are usually temporary and usually disappear once treatment is over. There are often ways to lessen these side effects. For example, there are drugs that can be given to help prevent or reduce nausea and vomiting. Be sure to ask your doctor or nurse about medicines to help reduce side effects, and let him or her know when you do have side effects so they can be managed effectively.

Some of these chemotherapy drugs also have side effects that are peculiar to them. For example, cisplatin can cause nerve damage. This nerve damage can sometimes lead to hearing loss or abnormal sensations in the hands and feet such as pain, burning or tingling sensations, hypersensitivity to cold or heat, or weakness. (This is called peripheral neuropathy.) In most cases this goes away once treatment is stopped, but it may last a long time in some people.

The standard treatments for Mesothelioma are similar to other cancers. Radiation and chemotherapy are definite musts. Surgical procedures are also very common. Still, these three more often than not fail to overcome the cancer and death often occurs within one year of diagnosis. All is not lost. There are a number of new therapies being researched. Let’s take a closer look.

Cancer is the mutation of the DNA in cells. Given this, Gene Therapy is a very new treatment being considered for use with Mesothelioma and most cancers. There are two approaches being investigated. The first is to replace the genetic structure of the cancerous cells and thus wipe out the cancer. The second approach is to use a knockout approach where genetic manipulation removes the genes that give rise to tumor growth. In theory, this would terminate tumor and cell growth and stop the cancer in its tracks.

Photodynamic therapy is as unique as it is hard to explain. There are certain chemicals that are very light sensitive. The body can absorb some of these chemicals into cells and then expel them. Cancerous cells, however, cannot expel the chemicals. A patient is given a dose and then the cells are exposed to a laser light. The chemicals kick out an active form of oxygen that destroys the cancerous cells. There is a downside, however. The patient becomes very light sensitive and often must stay out of the light for as long as six weeks.

Anti-angiogenesis treatment is another interesting area of treatment. It works by attacking the tumors created by Mesothelioma. It does this by eliminating the ability of the tumors to create blood vessels. Without such vessels, the tumors cannot get blood and cannot grow. Drugs such as Cisplatin, Pemetrexed and Bevacizumab are in clinical trials to determine if they can be used on Mesothelioma tumors.

Mesothelioma is a rare, but very serious form of cancer. While past treatment methodologies produced disappointing results, the new treatments being tried show some major promise. Make sure to discuss them with your oncologist and physician.

Mesothelioma is a cancer that is most often associated with exposure to asbestos. A diagnosis of Mesothelioma necessarily engenders a question of the possible treatments and outcome for the patient. The Butchart system is one way to gauge the stages of Mesothelioma.

Mesothelioma is like many cancers in that the variety of treatment options available changes with the progress of the disease. Catch it early and there are plenty of options to choose from. Catch it in the later stages and the prospects are not so positive. To understand the situation you or a friend who has been diagnosed with Mesothelioma are in, you need to understand the stages of the disease.

The Butchart System is the oldest staging system used for Mesothelioma. Oddly, there are others as well, but we will focus on the Butchart version in this article. The system is comprised of four stages and focuses on the primary tumor as a starting point. Let’s take a closer look.

Stage One

In stage one, the Mesothelioma has manifested in the pleura or lining of the chest. It is usually present in one side or the other. It may have also manifested in the diaphragm area as well.

Stage Two

The Mesothelioma has advanced significantly at this stage. It is now in the lining of both sides of the chest cavity. It may also be in the esophagus and lining of the heart. Lymph nodes in the chest are almost certainly impacted and may be creating a method of movement for the cancer.

Stage Three

The cancer continues to spread. It is now clearly into the esophagus and heart lining. It is also spreading throughout the Mesothelium of the abdomen. Lymph nodes beyond the immediate chest area are also showing cancer cells.

Stage Four

The Mesothelium has metastasized fully. It is moving through the blood stream to other parts of the body. Multiple internal organs are now showing cancer cells. The situation is very dire.

So, what treatment options apply to each of these stages? There really isn’t a pat answer. The problem is the treatment usually is very aggressive. It can involve any combination of surgery, radiation and chemo therapy. The exact mix will depend on your overall health and the opinion of your physicians.
